Minor Liability Waiver & Parental Consent

1. Acknowledgment of Risk

I understand that martial arts training involves physical contact and risks including:

I knowingly allow my child to participate.

2. Parental Assumption of Risk

I voluntarily assume all risks on behalf of my child, including those arising from negligence of YESUWAY MARTIAL ARTS LLC.

3. Release of Liability

I release and hold harmless YESUWAY MARTIAL ARTS LLC from any claims arising from my child’s participation.

4. Medical Authorization

I confirm my child is physically able to participate.

I authorize emergency medical care if needed and accept full financial responsibility.

5. Rules & Conduct

I understand my child must:

Failure may result in removal without refund.

6. Photo & Media Release 

I understand and acknowledge that photos, videos, and recordings may be taken of the minor participant during classes, training sessions, events, competitions, and other activities conducted by YESUWAY MARTIAL ARTS LLC.

As the parent and/or legal guardian of the minor participant, I grant YESUWAY MARTIAL ARTS LLC permission to photograph, record, and use the minor participant’s image, likeness, voice, and participation in photos, videos, digital media, promotional materials, advertisements, social media content, and other marketing or business-related purposes without compensation or further approval.

I understand and agree that all such media shall remain the sole property of YESUWAY MARTIAL ARTS LLC.

7. Acknowledgment

I certify that I am the legal parent or guardian and have authority to sign this agreement.

8. Parent Indemnification (CRITICAL)

I, as the parent/legal guardian, agree to indemnify and hold harmless YESUWAY MARTIAL ARTS LLC from any and all claims brought by or on behalf of my child, including claims for negligence.

9.  Covenant Not to Sue (Parent)

I agree that neither I nor my child will bring any legal action against YESUWAY MARTIAL ARTS LLC for injuries sustained during participation, to the fullest extent permitted by Georgia law.

10. Georgia Law & Venue

This agreement shall be governed by and interpreted in accordance with the laws of the State of Georgia. Any legal action shall be brought exclusively in the courts located within the State of Georgia.

11. Severability

If any portion of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

12. Acknowledgment of Understanding

I confirm that I have read and understand this agreement fully and sign it voluntarily on behalf of my child.
