1.  Acknowledgment of Risk

I understand that participation in martial arts training—including drills, sparring, conditioning, and use of equipment—involves inherent risks. These include, but are not limited to:

I voluntarily choose to participate with full knowledge of these risks.

2. Assumption of Risk

I knowingly and freely assume all risks, both known and unknown, even if arising from the negligence of YESUWAY MARTIAL ARTS LLC, its members, managers, instructors, employees, agents, or representatives.

3. Release of Liability

I hereby release, waive, and discharge YESUWAY MARTIAL ARTS LLC and its members, managers, instructors, employees, agents, and affiliates from any and all liability, claims, demands, or causes of action arising out of or related to any injury, disability, or loss that may occur during participation.

4. Medical Responsibility

I confirm that I am physically able to participate. I understand that YESUWAY MARTIAL ARTS LLC does not provide medical insurance.

I authorize emergency medical treatment if necessary and agree to be financially responsible for any medical services rendered.

5. Rules & Conduct

6. Photo & Media Release 

I understand and acknowledge that photos, videos, and recordings may be taken during classes, training sessions, events, competitions, and other activities conducted by YESUWAY MARTIAL ARTS LLC.

By participating, I grant YESUWAY MARTIAL ARTS LLC permission to photograph, record, and use my image, likeness, voice, and participation in photos, videos, digital media, promotional materials, advertisements, social media content, and other marketing or business-related purposes without compensation or further approval.

I understand and agree that all such media shall remain the sole property of YESUWAY MARTIAL ARTS LLC.

7. Acknowledgment

I have read, understood, and voluntarily agree to all terms.

8. Release for Negligence (Georgia-Specific)

I expressly agree that this Release of Liability includes any and all claims based on the negligence, action, or inaction of YESUWAY MARTIAL ARTS LLC, its members, managers, instructors, employees, agents, or representatives, to the fullest extent permitted by the laws of the State of Georgia.

9. Indemnification

I agree to indemnify, defend, and hold harmless YESUWAY MARTIAL ARTS LLC from any and all claims, damages, losses, liabilities, or expenses (including attorney’s fees) arising out of or related to my participation in activities.

10. Covenant Not to Sue

I agree that I will not initiate any lawsuit, claim, or legal proceeding against YESUWAY MARTIAL ARTS LLC for any injury or damages covered by this agreement.

11. Governing Law & Venue

This agreement shall be governed by and interpreted in accordance with the laws of the State of Georgia. Any legal action shall be brought exclusively in the courts located within the State of Georgia.

12. Severability

If any portion of this agreement is found to be invalid or unenforceable, the remaining provisions shall remain in full force and effect.

13. Entire Agreement

This agreement constitutes the entire understanding between the parties and supersedes any prior agreements or representations.
